How facelift treatment is done in Iran

As a person gets older some changes appear in the whole body. These changes include the depletion of fat in some parts and accumulation of it in some other parts, skin elasticity and firmness reduction in cheeks, eyelids and lower jaw and deepening of the smile line.
Face-lift includes cosmetic approaches to rejuvenate the appearance skin of the face by modifying facial skin and subcutaneous tissues and eliminating excess skin. However, removing the wrinkles that have been formed over a lifetime or due to photoaging needs to be treated through other medical approaches.

What are the face-lift methods?

There are different surgical and non-surgical face-lift methods. The chosen approaches for each patient are based on the experience of the surgeon, safety, efficacy, cost, age, and the needs plus expectations of facial rejuvenation. The doctor uses local anesthetic and sedatives or general anesthesia regarding the suitable approach. The preferable methods for young patients are non-surgical ones such as using ultrasound energy. Surgical methods mostly apply to those cases in which the non-surgical approaches are non-effective for elevating and tightening their skin and underlying tissues. There are different types of face-lift methods including:

The mini-face-lift

This method usually implements for the skin with minor wrinkles and laxity in the lower face. So, it is suitable for younger patients who desire to have well-defined skin.

The mid-face-lift

This method targets the mid-face region and sometimes it incorporates with other procedures. This surgical method focus on elevating the cheeks tissues along with lower eyelids and nasolabial skin improvement.

The deep plane face-lift

This procedure is usually recommended for patients with severe skin aging and laxity in cheeks and jaws. The outcome of lifting the skin tissues and correcting the wrinkly and loosen parts in this approach is the most natural-looking among other types of face-lift methods. Moreover, the new juvenile appearance remains at least for 5 years that is longer than other rhytidectomies.

The Silhouette Instalift

This is a mid and lower facial approach that is suitable for young patients who do not aim to have a full facial surgery and is known as a convenient in-office procedure. The Silhouette InstaLift is a minimally invasive method and is considered as a non-surgical cosmetic treatment. This is an efficient method with the least complications and recovery time. In the Silhouette InstaLiftusing approach, absorbable suture suspension is implemented for lifting and repositioning the tissues in different parts of the face such as jaws, cheeks, brows, and nose and lips creases.

Ultherapy

This is a non-surgical skin lifting and underlying tissues tightening method that is suggested for young patients to prevent skin laxity and keep the natural tissue consistency. Also, it is recommended to the patients with the history of face-lift approaches to prolong its results. Ultratherapy uses the ultrasound energy to help the skin of brows, chin, and jawline to get tighten.

What are the pre-operation considerations?

During the pre-operation phase, the patient has appointments to consult with a doctor about the expectations and goals of the patient. The doctor considers these expectations with the physical condition of the patient and matches them with the best medical approach. It should be noted that the chance of complications in patients with overweight, a history of diabetes, chronic obstructive pulmonary disease (COPD), uncontrolled hypertension, alcohol abuse, bleeding disorder, keloids, or smoking history is higher. Also, those people with a history of taking prophylactic aspirin and anticoagulants, platelet inhibitors, isotretinoin, or dietary supplements that impact coagulation are at risk for complications. Furthermore, a list of avoidable medications is usually given to the patients before the operation. Before the operation, the heart, lungs, blood pressure, pulse, and temperature are checked, and the patient is asked further questions, such as NPO status, possible pregnancy if a female, and the discontinuance or ingestion of medications as previously instructed.

How long does the face lift surgery take?

It depends on the procedure that is applied and whether other parts like the neck is also included or not. Nonetheless, the whole surgery procedure takes between 2 to 5 hours.

How long does this new appearance remain after rhytidectomy?

Face-lift surgery gives a new look to oneself. Yet, this may not last longer than 10 years and after this period the skin gets loose and saggy again.

What are the complications of face-lift surgery?

The complications in this surgery would be limited if the surgeon is professional and highly experienced. However, there might be some unforeseen and unavoidable complications in any surgery.
These may include:
Bleeding
Bruising
Pain
Complications of anesthesia
Damage to the facial nerves controlling muscles (usually temporary)
Skin slough (uncommon)
Hematoma
Ectropion
Infection
Alopecia around the incision site (uncommon)
Numbness (which can improve within days or weeks)
Scarring
Skin necrosis, or tissue death
Asymmetry
Ear Deformities
Widening or thickening of scar
It should be noted that the combination of face-lift surgery with other procedures increases the risk and complication possibility.

What are the post-operative cares?

Some discomfort such as mild to moderate pain, tenderness, swelling, and numbness may occur after surgery that can be relieved by some prescribed medications. However, a doctor should be seen immediately in the case of experiencing intense symptoms (especially in the first 48 hours post-operation) like chest pain, arrhythmia, short breathing or severe pain.
The patient should follow the instructions of the doctor about a healthy lifestyle and diet, wound care, bathing, and washing plus avoidance of smoking, alcohol intake, sun exposure, stress, wearing makeup, hair coloring or bleaching, intense exercises, and pollutants. The doctor removes the sutures approximately after a week and the patient would recover after 2 to 4 weeks.
